Overview
Provides professional nursing care for patients following established standards and practices, which can include triage (telephone and walk‑in visits) and care needs that transpire between office visits. The position ensures timely access to appropriate services, reduces care fragmentation, and promotes optimal health outcomes and patient safety. Demonstrates professional clinical leadership and oversight within the scope of the RN role.
Responsibilities
- Triage patient needs via telephone and during visits to ensure timely access to appropriate level of care, reducing avoidable ED visits and readmissions in accordance with IHA policies and protocols.
- Conduct post‑discharge calls for patients undergoing transition of care for identified populations.
- Document clinical findings in a clear, concise, and timely manner as required by IHA policy.
- Use critical thinking within licensure scope to assess patient needs and involve providers as appropriate.
- Enter orders (medication, laboratory, radiology, referrals) per provider direction and applicable guidelines.
- Follow relevant clinical and nursing protocols.
- Schedule patient appointments as needed.
- Encourage usage of the patient portal and provide support where needed.
- Provide scope‑appropriate nursing education during all patient interactions.
- Consult with providers and extended care team members (Care Management, Embedded Pharmacy, Behavioral Health, Nutrition) and external case managers to ensure quality patient care.
- Support physicians and advanced practice providers with indirect work according to protocol (e.g., test results/orders management).
- Assess patients for gaps in care for preventive and disease‑management needs.
- Ensure patient medication list is accurate, complete medication reconciliations, and process refill requests per protocol.
- Co‑manage indirect patient work with the care team in accordance with standard work instructions.
- Collaborate with insurance, DME, home care, and other care partners to meet patient needs.
- Review patient medical history to identify supporting documentation for ordered services and procedures to assist with or complete prior authorization processes.
- Assist patients with resources for social influencers of health needs.
- Participate in process‑improvement projects within the office/department.
- Serve as a resource to clinical office/department staff within the RN scope; may direct the work of LPN and medical assistant staff as needed and appropriate.
- Perform clinical support tasks as outlined in the Clinical Support Staff Scope of Practice document.
- Support other offices, attend required meetings and training, and participate in committees as requested.
- Attend weekly patient‑care meetings and daily huddles as applicable.
- Assume additional duties as required.

Qualifications
- Graduate of an accredited Registered Nurse program.
- Valid, unrestricted RN license in the State of Michigan and valid CPR certification.
- 1‑2 years of clinical experience in a medical or physician office (preferred; previous nursing experience not required).

- Basic knowledge of the relevance of vital statistics to patient health history.
- Proficient in medical terminology.
- Proficient with desktop Windows systems, including electronic medical records, email, intranet, and other required software.

- Ability to travel to other office/practice sites for meetings and training.
- Successful completion of IHA competency‑based program during the introductory and training period.
Physical Requirements
- Keyboarding, phone work, and charting.
- Time working on a computer.
- Lifting over 50 lbs.
- Handling and lifting patients; transferring to gurney or wheelchair; assisting with dressing, walking, bending, stooping, reaching, climbing, kneeling, or twisting as needed.
- Pulling, pushing, or lifting up to 100 lbs.
- Close vision, depth perception, color vision, peripheral vision, and focus adjustment.
- Manual dexterity for keyboard, photocopier, telephone, calculator, and other office equipment.
- Hearing and speaking sufficient for phone and face‑to‑face communication in English.
Environmental Requirements
This job operates in a medical facility and requires regular walking to various locations around the hospital/clinic. Employees work where patient care equipment is present. Hazardous materials, including bloodborne pathogens and bodily fluids, are also present. Exposure to sharps, x‑rays, patients’ conditions, and some unpleasant sights, smells, and contagious diseases are possible. This position requires significant interaction with people who may be scared, hurt, and/or ill, which can be stressful and result in competing priorities.
