Job summary

Everguide Health is seeking a dedicated Registered Nurse to serve as the clinical backbone of our care pod. You will travel to members' homes, translate physician directions into action, and own how you show up for them with autonomy and careful judgment.

You will build lasting relationships with a small member panel, coordinate care with specialists, perform phlebotomy and point-of-care testing, and document data in the EHR while advancing preventive health plans over time.

Qualifications

• Associate’s or Bachelor’s degree in Nursing.
• Minimum 4 years of clinical experience.
• Experience in care coordination or concierge-style settings preferred.
• Full-time (40 hours/week) with flexible scheduling.
• High autonomy in managing schedule and member relationships.

What You Will Do

The Registered Nurse serves as the clinical backbone of the care pod. You will translate physician direction into action, ensure follow-through across diagnostics and care plans, and act as a consistent point of contact for members. This role comes with real autonomy: you'll build direct, lasting relationships with a small panel of members and largely own how you show up for them – in their homes, at their pace, on your clinical judgment. You are not only delivering care – you are stewarding it, and you'll see the impact of your coaching and nursing skill unfold over months and years, not just a single visit.

Clinical Care & Member Advocacy

• Travel locally to meet members in their homes, offices, and other appropriate community settings.
• Be comfortable entering members’ homes and work environments as a trusted clinical professional.
• Communicate results clearly and efficiently, escalating when appropriate.
• Coordinate with specialists and external providers to maintain continuity of care.
• Perform assessments, phlebotomy (adult and pediatric), and point-of-care testing.
• Monitor and document clinical data accurately in the EHR, anticipating the needs of members and closing loops without being prompted.
• Reinforce preventive strategies and long-term health plans, and build the kind of trust that lets you coach members toward real, lasting change.
• Advocate for members within the healthcare system when coordination or access is required.
• Participate in developing and refining clinical workflows and policies.
• Protect confidentiality and uphold the highest professional standards.
