Job summary

JourneyCare Home Health in Arlington Heights, IL is seeking an experienced Registered Nurse to manage a home health caseload. You will coordinate care with an interdisciplinary team and perform skilled nursing procedures in patients’ homes.

The role requires IL RN licensure, CPR certification, and 1 year of clinical nursing experience (home health preferred). Travel is required, with strong emphasis on patient-centered care and coordination.

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• At least one year clinical nursing experience; home health preferred.
• CPR certification required; reliable transportation and valid driver's license.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment, eligibility, admissions processing, planning, implementation and evaluation of the nursing process.
• Assess data from patient’s physical, psychological, social and spiritual factors and coordinate interventions with the interdisciplinary team.
• Initiate communication with the attending physician and team to coordinate optimal care.
• Provide visits to assess patient needs and update the plan of care.
• Perform nursing procedures as directed by the physician.
• Perform clinical pronouncement of death and provide family support as licensure allows.
• Maintain accurate patient records and timely care plans.

What You�ll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Graduate from an accredited registered nursing program and licensed in the state
• At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
• Home health experience preferred
• Possess and maintain valid CPR certification.
• Must have reliable transportation, current driver's license and appropriate automobile insurance
