Job summary

Trinity Health IHA Medical Group in Michigan is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will perform clinical triage, support care coordination, educate patients, and assist with transitions of care to improve outcomes and patient experience.

Responsibilities include triage by phone and in-office visits, coordinating with providers and pharmacists, managing referrals and prior authorizations, and helping patients access community resources.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in Michigan.
• Valid CPR certification.
• 1-2 years of clinical experience in a medical or physician office preferred.

• MINIMUM EXPERIENCE: 1-2 years of clinical experience in a medical or physician office preferred.
