Job summary

TLC Care Services in Anchorage, Alaska, seeks a compassionate RN to deliver high-quality home health nursing care. You will work in patients' homes, conducting assessments, administering medications, and updating care plans while collaborating with the care team.

Qualifications

  • Strong competency in clinical assessment and critical thinking

Responsibilities

  • Provide skilled nursing care to patients in their homes
  • Conduct patient assessments and monitor health status
  • Administer medications and perform wound care, injections, and other ordered treatments
  • Develop, implement, and update patient care plans
  • Complete required clinical documentation, including OASIS when applicable
  • Report significant changes in patient condition to the care team and physician
  • Coordinate care with physicians, therapists, and other healthcare professionals
  • Maintain a safe environment during home visits
  • Provide clinical guidance for complex cases and care decisions
  • Review and sign care plans as required
  • Support development and implementation of nursing protocols and clinical procedures, as authorized
  • Ensure compliance with state and federal home health regulations

Hourly Rate: $40-$48
Job Type: Full-time / Part-time / On-Call
