Position Overview

The Registered Nurse (RN) provides and coordinates nursing care for residents while supporting their health, safety, comfort, dignity, and quality of life.

The RN is responsible for resident assessment, medication and treatment administration, changes in condition, clinical documentation, implementation of care plans, communication with providers and families, and clinical oversight of the nursing team.

As a Registered Nurse, you will also serve as a clinical resource for LPNs, CNAs, and other direct-care team members while helping ensure residents receive safe, timely, and coordinated care.

Key Responsibilities

Resident Assessment & Clinical Care

• Assess and monitor residents according to their individual clinical needs

• Provide nursing care in accordance with provider orders, resident care plans, facility procedures, and applicable scope of practice

• Monitor residents for changes in physical, cognitive, behavioral, and emotional condition

• Respond promptly to changes in condition and clinical concerns

• Obtain and evaluate vital signs and other clinical information

• Perform nursing treatments and interventions as appropriate

• Monitor residents with acute or changing clinical needs

• Respond appropriately to resident emergencies

• Promote resident comfort, dignity, independence, and quality of life

Medication & Treatment Management

• Administer medications and treatments safely and accurately

• Follow provider orders and established medication-administration procedures

• Monitor residents for medication effectiveness, side effects, and adverse reactions

Clinical Leadership & Shift Oversight

• Serve as a clinical resource for LPNs, CNAs, and other nursing team members

• Provide direction and delegation within applicable scope of practice and facility procedures

• Help ensure resident-care assignments and nursing responsibilities are complete

• Promote accountability, teamwork, and professional communication

• Identify documentation or care concerns requiring additional follow-up

Resident & Family Communication

• Build professional and compassionate relationships with residents and families

• Communicate appropriate information regarding resident care and condition

• Respond professionally to resident and family questions or concerns

Quality & Resident Outcomes

• Promote high-quality nursing care and positive resident outcomes

• Monitor residents at increased risk for falls, wounds, infections, weight changes, hospital transfers, and other clinical concerns

• Identify trends or changes requiring additional nursing intervention

• Participate in clinical audits and quality-improvement initiatives as assigned

• Support implementation of corrective actions and resident-specific interventions

• Promote proactive identification of resident-care concerns

• Assist nursing leadership with maintaining ongoing clinical and survey readiness

Qualifications

• Active New Mexico Registered Nurse (RN) license in good standing or other applicable authorization to practice nursing in New Mexico

• Graduate of an approved nursing program

• Strong resident-assessment and clinical decision-making skills

• Ability to recognize and respond appropriately to changes in resident condition

• Strong medication-management and clinical documentation skills

• Effective communication and interpersonal skills

• Ability to provide clinical direction and support to nursing team members

• Dependable, organized, compassionate, and resident-focused

• Skilled nursing, long-term care, rehabilitation, post-acute, senior living, or related nursing experience preferred

• Experience working with older adults preferred

• New graduate RNs are encouraged to apply
