Job summary

Phaxis seeks an experienced RN to provide in-home health nursing care in the Albany, KY area. The role includes assessing patients, developing and implementing care plans, and coordinating with a multidisciplinary team to support health and safety in community settings.

The RN will supervise plan implementation, monitor medications, and ensure documentation is up to date, while working a Monday–Friday schedule with on-call duties.

Qualifications

• RN license required for Kentucky.
• Experience in Home Health or community health settings preferred.
• Proficient in IV, wound care, and medication administration.

Job description

We are seeking a dedicated professional to join our healthcare team. The candidate will be responsible for assessing the health of individuals and implementing their care plans, ensuring health maintenance, disease prevention, and providing case management. Supervision of care plans and staff members may also be involved in daily work assignments.

Shift Requirements

• Monday-Friday 8am-5pm
• On Call Schedule: 1-2 days during the week and may have a weekend a month

Licensing and Requirements

• RN compact state license required
• Home Health experience, HCHB and OASIS experience required
• Skills in IV, wound care, PICC line, wound vac, Trach, lab draws

Responsibilities

• Observe, assess, plan, implement, and evaluate nursing support for well, chronic, or acutely ill individuals in a community or small facility setting
• Work closely with other health professionals/providers in planning, implementing, and evaluating programs and services
• Coordinate communications and actions of various disciplines as it relates to health and safety supports
• Ensure the ordering, reviewing, monitoring, discontinuation, and, if necessary, the disposal of medications as prescribed by the physician
• Ensure policies are in place for medication administration, errors, storage/access, and destruction, and review them annually
• Report progress to the physician when the client’s condition changes
• Transcribe or ensure transcription of physician phone orders and other medically related reports
• Complete daily, monthly, quarterly, and annual nursing documents as required
• Document individual’s health status in nurse’s notes
• Maintain furnishings, equipment, and supplies to prevent the spread of disease
• Direct and supervise the activities of licensed practical/vocational nurses as needed
• Schedule and complete health-related appointments and implement appropriate responses
• Maintain communication with inpatient nursing and discharge coordinators
• Provide new employee training and annual recertification training for health and safety topics
• Train unlicensed staff on person-specific health maintenance, safety, and disease prevention needs
