Job summary

Professional Case Management seeks a compassionate Registered Nurse to deliver high-quality in-home care to clients with complex medical needs, including former energy workers. You will perform comprehensive assessments, administer treatments and medications, manage wound and IV care, and coordinate with an interdisciplinary team to develop and implement individualized care plans.

You will educate patients and families, ensure accurate documentation, and follow evidence-based practices.

Qualifications

• Valid RN license in good standing.
• Experience in home health is preferred.
• Strong documentation and patient education skills.
• Ability to work independently and as part of a team.

Responsibilities

• Perform comprehensive assessments in clients' homes.
• Administer treatments and medications as prescribed.
• Manage wound and IV care.
