Holliswood Center is seeking a Registered Dietitian (RD) in Hollis, NY to join our skilled nursing team. The role offers a $5,000 sign-on bonus and requires RD or RD Eligible status, with a focus on resident nutrition assessment and care planning.

You will work closely with medical staff and the interdisciplinary team to develop and monitor nutrition care plans, educate residents and families, and ensure compliance with state and federal guidelines.

Formación

• Must be an RD or RDE.
• Degree in Nutrition or Dietetics.
• Exceptional interpersonal & leadership skills.
• Knowledge of applicable state and federal guidelines.
• Computer literacy and proficiency with EMR software and computer-based menu systems.

Responsabilidades

• Assess residents' nutritional status and develop care plans.
• Evaluate nutrition needs for weight issues and skin breakdown.
• Coordinate procurement of nutrition supplies with vendors and department heads.
• Monitor meal service to ensure diet modifications are followed.
• Educate residents, families and staff on nutrition concepts.
• Routinely evaluate care plans for effectiveness and quality.
• Meet weekly with interdisciplinary team to review plans.
• Collaborate with Speech-Language Pathologist on dysphagia diets.
• Ensure menu changes comply with policies and federal/state guidelines.
• Update diet orders and menu changes daily.
