Job summary

LaSante Health Center in Brooklyn, NY is seeking a knowledgeable Registered Dietitian (RD/RDN) to join our healthcare team. This full-time on-site position focuses on individualized nutritional counseling, weight management, and chronic disease prevention and management within a diverse patient population.

You will collaborate with medical staff to create personalized meal plans, educate patients, and monitor progress.

Qualifications

RD/RDN credential required and active license/registration.
Experience in dietetics, nutritional counseling, and education.
Knowledge of weight management and nutrition-related chronic diseases.
Ability to engage diverse patient populations and communicate clearly.

Responsibilities

Conduct nutritional assessments and tailor dietary plans to individual goals.
Provide one-on-one counseling and education to patients.
Develop personalized meal plans based on medical conditions and lifestyle.
Support weight management and management of diabetes, hypertension, and cardiovascular risk.
Educate on healthy food choices, portion control, and sustainable lifestyle changes.
Collaborate with physicians, nurses, and other health professionals for comprehensive care.
Monitor progress and adjust plans as needed.
Maintain accurate patient documentation.
Deliver culturally sensitive nutrition education.
