The role involves nutrition assessments, counseling, and collaboration with interdisciplinary teams to improve residents’ health, with emphasis on compliance and ongoing professional development.

Qualifications

• Must be a Registered Dietitian or RD eligible.
• Strong organizational and communication skills.
• Experience in LTC and EMR preferred.

Responsibilities

• Perform nutritional assessments and ongoing evaluation of the plan of care for each resident.
• Participate in IDCP care conferences as needed.
• Provide nutrition education and counseling to residents, families, and staff as needed.
• Collaborate with interdisciplinary teams to improve residents' health and well-being.
• Completes other duties as assigned.
• Ensure compliance with federal, state, and local regulations and guidelines.
