You will lead nutrition care across inpatient and outpatient settings, build programs, and collaborate with clinicians and food service staff to improve outcomes.

Responsibilities

• Lead nutrition care: assessment, care planning, monitoring, education, and counseling.
• Develop outpatient services: MNT, IBT, DSME.
• Provide diabetes education in collaboration with disease management program.
• Collaborate in plan-of-care meetings as patient advocate.
• Deliver inpatient nutrition services as needed and improve outcomes.
