You’ll perform nutrition assessments, educate patients, monitor status, and collaborate with a multidisciplinary team.

Qualifications

• Nutrition assessments and patient care planning.
• Educate patients regarding nutrition to promote wellness.
• Monitor weight, nutritional status, wounds, and nutrition support needs, including tube-feeding management when applicable.
• Collaborate with RDs, Physicians, Speech & Occupational Therapists.
• Help improve patient outcomes using “Food as Medicine”.

• Educate patients about nutrition and wellness strategies.
• Monitor weight, nutritional status, wounds, and nutrition support needs.
• Coordinate with RDs, physicians, and therapists.
