Identifies nutrition related issues and obtains assistance/referrals for complex problems. Writes therapeutic diet orders within scope of practice.4. Monitors patients' progress.5. Communicates effectively with patients, family, and all medical staff. Provides appropriate information (problems, requests for service, improvement suggestions, etc.) to management.6. Educates and provides guidance to patient, family, care-givers, and staff on diet, feeding regimen or other nutritional information prior to discharge or during outpatient consultation.7. Functions as a part of a multidisciplinary medical team. Collaborates with nutritional services to ensure that necessary foods are available and procures formula from appropriate sources. Works with dietetic interns and provides peer review of nutritional assessments and diet orders.8. Documents all pertinent nutrition information in patient's medical records according to policy. Document daily charges for patients seen, procedures and activities performed and time spent on each.9. Attends nutritional case studies, department in-service and other training opportunities and document continuing education credit in CDR format.10. Actively participate in Medical Team rounds.11.
