Qualifications

• Registered by the Commission on Dietetic Registration.
• State license in good standing required.
• Knowledge of state survey process and regulations preferred.
• Strong organizational and communication skills.
• COVID-19 vaccination policy compliance as required.
• If multi-site, valid driver's license.

Responsibilities

• Assess residents' nutrition needs and document plans in the EMR.
• Develop and evaluate individualized care plans from findings and labs.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure adherence to CMS, state and federal regulations and standards.
• Provide coverage across multiple buildings as needed.
• All other duties as assigned.
