Qualifications

• Minimum 2 years in acute care, rehab, or long‑term care.

Responsibilities

• Conduct patient assessments to identify nutritional needs and develop personalized meal plans.
• Collaborate with healthcare team to coordinate care and communicate findings.
• Provide nutritional counseling and education to patients and families.
• Monitor progress and adjust nutrition plans as needed.
• Develop and implement nutrition programs to promote patient well‑being.
• Participate in quality improvement initiatives to improve outcomes.
• Maintain accurate records of care and nutrition plans.
