Qualifications

Responsibilities

• Develop and implement personalized nutrition plans for residents with dietary needs.
• Conduct initial and ongoing assessments of residents’ nutritional status and create individualized care plans.
• Collaborate with the healthcare team to ensure comprehensive care.
• Provide education to residents, families, and caregivers on nutrition topics.
• Participate in quality improvement initiatives to improve resident outcomes and satisfaction.
• Maintain accurate records of resident care and treatment.
