Qualifications

• B.S. Degree in Foods and Nutrition or advanced degree in nursing, nutrition, or a related field.
• Registered dietitian training with six months experience; preferred 2 years in general practice.
• Eligible for registration with the Academy of Nutrition and Dietetics.
• CDCES/CDE preferred; certification targeted within 12 months with organizational support.
• Active BLS certification within 30 days of hire.

Responsible for providing comprehensive education and support to individuals and their families with diabetes, obesity, and any metabolic/endocrine associated disorder, helping them to manage their condition and achieve their health goals. Responsible for providing comprehensive education and support to individuals and their families with diabetes, obesity, prediabetes, and metabolic/endocrine disorders. The position supports individual patient outcomes while also leading population health initiatives focused on improving quality measures, reducing complications, optimizing diabetes control, and addressing obesity-related health risks across the communities served by the Hospital Authority of Miller County.

EDUCATION, CREDENTIALS & EXPERIENCE REQUIREMENTS:

• B.S. Degree in Foods and Nutrition or Advanced degree in nursing, nutrition, or a related field.
• Successful completion of Dietetic Training Program with six-months experience under a registered dietitian. Preferred 2 years of professional general practice experience.
• Eligible for registration with the Academy of Nutrition and Dietetics.
• Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE) certification preferred. Candidates with a strong clinical background and demonstrated experience in diabetes management, diabetes education, endocrinology, metabolic disease management, population health, or chronic disease management will be strongly considered. Employees who do not currently hold CDCES/CDE certification will be expected to obtain certification within twelve (12) months of eligibility, with organizational support provided for certification attainment.
• Active American Heart Association BLS certification within 30 days of hire.

JOB SPECIFIC COMPETENCIES:

• Reviews, revises, and participates in the development of the facility’s patient care policies to ensure the inclusion and support of nutritional care.
• Assures that quality procedures are appropriate for collecting and summarizing.
• Recommends suitable clinical and administrative references pertaining to nutrition.
• Collaborates with physicians, mid-levels, and clinical staff to identify and provide appropriate nutritional counseling.
• Establishes treatment plans and communicates with physicians/mid-levels regarding nutrient needs.
• Meets directly with patients to identify educational needs, provide individual instruction, utilize informational handouts/booklets, or refer to community resources as needed.
• Establish priorities, goals and objectives which meet nutritional needs and are consistent with available resources.
• Responsible for corrective action on any deficiencies or recommendations from health agencies. i.e.: Division of Human Resources, DNC, etc.
• Will serve as a liaison to industry representatives to assure that the clinic is properly equipped and utilizing technology inherent in the treatment of patients with diabetes or other applicable conditions.
• May participate in outreach activities as defined through strategic growth initiatives for the practice.
• Perform other duties as assigned. Must be HIPAA compliant.
• Develop and oversee diabetes and obesity population health programs designed to improve clinical outcomes, patient engagement, and quality measures.
• Track, analyze, and report key performance indicators including A1C control, blood pressure control, continuous glucose monitor utilization, diabetes education completion, weight reduction, obesity management outcomes, preventative screenings, and hospital utilization trends.
• Utilize electronic health records, registries, and quality dashboards to identify care gaps and target patients requiring intervention.
• Collaborate with primary care providers, endocrinologists, pharmacists, behavioral health providers, and community partners to improve overall population health outcomes.
• Develop strategies to improve performance in diabetes-related quality metrics and obesity management measures.
• Coordinate and implement evidence-based weight management and lifestyle intervention programs focused on nutrition, physical activity, behavior modification, and chronic disease prevention.
• Establish measurable goals and benchmark outcomes for diabetes management, obesity reduction, cardiovascular risk reduction, and overall population health improvement.
• Prepare monthly and quarterly reports demonstrating program effectiveness, patient outcomes, quality measure performance, and return on investment for diabetes and obesity management initiatives.
• Participate in community outreach, wellness programs, and educational events focused on diabetes prevention, obesity reduction, healthy lifestyle promotion, and chronic disease management.
