Job summary

Healthcare Services Group, Inc. is seeking a Registered Dietitian to join our clinical nutrition team at a long-term care facility. You will assess residents' nutrition needs, develop care plans, and document them in the EMR, delivering high-quality nutrition care.

Collaborate with interdisciplinary colleagues to improve residents' health outcomes while complying with CMS, state, and federal regulations. Flexibility with schedules and coverage across buildings is available.

Qualifications

• Registered Dietitian credential required (RD)
• State license in good standing
• Knowledge of state survey process and compliance (preferred)
• Strong organizational and communication skills
• Flexible, multi-site coverage and COVID-19 vaccination policy compliance
• RD-eligible and provisional licensure accepted in some locations

Responsibilities

• Assess residents' nutrition needs and document care plans in the EMR
• Develop, implement, and evaluate individualized nutrition plans based on clinical data
• Provide nutrition education and counseling to residents, families, and staff
• Collaborate with interdisciplinary teams to improve health outcomes
• Ensure compliance with CMS, state and federal regulations and company standards
• Provide coverage across multiple buildings as needed
• Perform other duties as assigned

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop and document the plan within the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities
• Provide nutrition education and counseling to residents/patients, families, and staff.
• Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ overall health and well-being.
• Ensure adherence to CMS, State, and Federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs, and as desired.
• All other duties as assigned.

Qualifications

• Registered by the Commission on Dietetic Registration. **
• Hold a Certification/License in good standing within the state of practice.
• Knowledge of the state survey process and compliance with nutrition care regulations (preferred, but not required).
• Strong organizational and communication skills.
• Maintain a positive attitude and effectively communicate with building occupants, administration, and HCSG leadership to perform daily tasks.
• Compliance with COVID-19 vaccination policies as required by state, local, and/or customers.
• If multi-site registered dietitian, must have valid Driver's License
• RD-Eligible and provisional state licensure accepted in certain locations
