Job summary

New Horizon Foods, Inc. is seeking a skilled Dietitian to provide nutrition care plans for seniors in a Skilled Nursing setting. The Dietitian will collaborate with other healthcare professionals to ensure residents receive personalized, evidence-based nutrition and ongoing care.

Work is hybrid: 2 days on site in Minneapolis with the rest able to be completed remotely. Compensation ranges from $30 to $32 per hour, with flexible hours and a supportive, mission-driven team focused on serving the

Qualifications

• Proven experience as a Dietitian.
• Experience working with seniors in a senior living environment.
• Strong knowledge of nutrition and healthy eating habits.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Develop and implement nutrition care plans for senior residents in multiple locations.
• Assess the nutritional needs of residents and provide appropriate recommendations.
• Collaborate with other healthcare professionals to ensure that residents receive comprehensive care.
• Educate residents and their families on proper nutrition and healthy eating habits.
• Monitor and evaluate the effectiveness of nutrition care plans.
• Maintain accurate and up-to-date records of resident nutrition assessments and care plans.
