Job summary

Healthcare Services Group, Inc. in Michigan seeks a Registered Dietitian to lead clinical nutrition services for a long-term care facility. Monday to Friday with flexible schedules, joining a team dedicated to improving resident care and outcomes.

The role involves assessing nutrition needs, creating care plans, educating residents and staff, collaborating with interdisciplinary teams, and ensuring CMS/state regulation compliance. Travel across buildings may be required.

Qualifications

• Registered dietitian with current licensure in the state of practice.
• Certification/licensing in good standing required.
• Knowledge of state survey processes and nutrition regulations is preferred.
• Strong organizational and communication skills are essential.
• Maintain a positive attitude and effectively communicate with staff and leadership.
• COVID-19 vaccination policies may apply where required.
• RDs working across multiple sites may require a valid driver's license.
• RD-Eligible and provisional licensure accepted in certain locations.

Responsibilities

• Assess residents' clinical nutrition needs and document plans in the EMR.
• Develop, implement, and evaluate individualized care plans based on findings, labs, weight trends, intake and comorbidities.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve residents' health and wellbeing.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings as needed.
• All other duties as assigned.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop and document the plan within the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities
• Provide nutrition education and counseling to residents/patients, families, and staff.
• Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ overall health and well-being.
• Ensure adherence to CMS, State, and Federal regulations and company standards.

Qualifications

• Registered by the Commission on Dietetic Registration. **
• Hold a Certification/License in good standing within the state of practice.
• Knowledge of the state survey process and compliance with nutrition care regulations (preferred, but not required).
• Strong organizational and communication skills.
• Maintain a positive attitude and effectively communicate with building occupants, administration, and HCSG leadership to perform daily tasks.
• Compliance with COVID-19 vaccination policies as required by state, local, and/or customers.
• If multi-site registered dietitian, must have valid Driver's License
• RD-Eligible and provisional state licensure accepted in certain locations
