Job summary

Regency on the Lake, part of Ciena Healthcare in Fort Gratiot, MI, seeks a compassionate Registered Dietitian to support residents in skilled nursing, rehab, and memory care. You will assess nutritional needs, develop individualized care plans, manage therapeutic diets, and collaborate with nursing, rehab, and medical staff to promote optimal health outcomes.

You will educate residents and families, ensure regulatory compliance, and support quality improvement.

Qualifications

• Current Registered Dietitian credential through CDR.
• Michigan state licensure eligibility or active licensure.
• Completion of accredited dietetics program with supervised practice.
• Experience in skilled nursing/rehab/long-term care preferred.
• ServSafe or food safety certification preferred.

Responsibilities

• Conduct comprehensive nutritional assessments for residents in skilled nursing, rehab, and memory care settings.
• Develop and implement individualized nutrition care plans and therapeutic diets.
• Monitor residents’ nutritional status and adjust plans based on clinical changes.
• Collaborate with nursing, rehabilitation, and medical teams on interdisciplinary care plans.
• Provide nutrition education and counseling to residents, families, and caregivers.
• Ensure compliance with federal, state, and facility nutrition and food service regulations.
• Document assessments, care plans, and progress notes accurately and timely in the EMR.
• Participate in quality improvement initiatives related to nutrition and dining services.
• Work with dining services to review menus and support safe, appealing meal options.
• Contribute to a resident-centered culture emphasizing respect, dignity, and well-being.
