Healthcare Services Group (HCSG) is seeking a Registered Dietitian to manage clinical nutritional services in a long‑term care facility. You will assess residents' nutrition needs, develop care plans, and document outcomes in the EMR, contributing to high-quality resident care.

This role emphasizes collaboration with interdisciplinary teams and compliance with CMS/state regulations. This position requires coverage across multiple buildings as needed.

Qualifications

• Registered dietitian with active RD credential
• Licensed in applicable state and in good standing
• Strong organizational and communication skills
• Knowledge of CMS/state/federal nutrition regulations
• Willingness to cover multiple buildings as needed

Responsabilités

• Assess residents' clinical nutrition needs and document plans in EMR.
• Develop, implement, and evaluate individualized nutrition care plans.
• Provide nutrition education and counseling to residents, families and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs.
• All other duties as assigned.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop and document the plan within the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities
• Provide nutrition education and counseling to residents/patients, families, and staff.
• Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ overall health and well‑being.
• Ensure adherence to CMS, State, and Federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs, and as desired.
• All other duties as assigned.

Qualifications

• Registered by the Commission on Dietetic Registration. **
• Hold a Certification/License in good standing within the state of practice.
• Knowledge of the state survey process and compliance with nutrition care regulations (preferred, but not required).
• Strong organizational and communication skills.
• Maintain a positive attitude and effectively communicate with building occupants, administration, and HCSG leadership to perform daily tasks.
• Compliance with COVID-19 vaccination policies as required by state, local, and/or customers.
• If multi-site registered dietitian, must have valid Driver's License
• RD-Eligible and provisional state licensure accepted in certain locations
