Job summary

Medical Nutrition Therapy Associates, LLC seeks a Registered Dietitian for a busy LTC facility in Chicago. The position can be part-time or full-time with competitive pay and comprehensive benefits.

You will perform nutritional assessments, educate residents and families, and collaborate with the care team to improve health outcomes. LTC and EMR experience is preferred.

Qualifications

• Must be a Registered Dietitian or RD eligible.
• Strong organizational and communication skills.
• Experience in LTC and EMR preferred.

Responsibilities

• Perform nutritional assessments and ongoing evaluation of the plan of care for each resident.
• Participate in care conferences as needed.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve residents' health and well-being.
• Ensures compliance with federal, state, and local regulations and guidelines.
• Complete other duties as assigned.

Job Responsibilities include:

• Perform nutritional assessments and ongoing evaluation of the plan of care for each resident
• Participate in IDCP care conferences as needed
• Provide nutrition education and counseling to residents, families, and staff as needed.
• Collaborate with interdisciplinary teams to improve residents' overall health and well-being
• Demonstrates ability to work in a cooperative manner as a team member
• Completes other duties as assigned.
• Ensure compliance with federal, state, and local regulations and guidelines
  

Job Requirements:

• Candidate must be a Registered Dietitian or RD eligible
• Candidate must possess organizational skills, as well as excellent communication and interpersonal skills
• Experience in LTC and EMR preferred
