This role involves providing high-quality nutrition care for residents in LTC facilities, SNFs, and adult medical daycares. Travel to visit and oversee facilities is expected as part of the regional scope.

Qualifications

• Registered Dietitian with current licensure.
• Experience in long-term care or EMR systems preferred.
• Willingness to travel to oversee facilities.

Responsibilities

• Perform nutritional assessments and ongoing evaluation of care plans.
• Participate in care conferences as needed.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve resident health.
• Train and audit other dietitians.

Job Responsibilities include:

• Perform nutritional assessments and ongoing evaluation of the plan of care for each resident
• Participate in IDCP care conferences as needed
• Provide nutrition education and counseling to residents, families, and staff as needed.
• Collaborate with interdisciplinary teams to improve residents' overall health and well-being
• Demonstrates ability to work in a cooperative manner as a team member
• Completes other duties as assigned.
• Ensure compliance with federal, state, and local regulations and guidelines
• Train and audit other dietitians

Job Requirements:

• Candidate must be a Registered Dietitian
• Candidate must possess organizational skills, as well as excellent communication and interpersonal skills
• Experience in LTC and EMR preferred
• Candidate must be willing to travel to visit and oversee facilities
