Legacy Healthcare invites applicants for the Regional Nurse Consultant position in South Dakota. This role provides clinical leadership, regulatory oversight, and operational support to multiple long-term care facilities, partnering with Administrators, Directors of Nursing, and interdisciplinary teams to ensure compliant, resident-centered care.

The successful candidate will be a SD-licensed RN with 10+ years in LTC, 5+ years as a DON in a SNF, and 3+ years RNC experience in LTC.

Qualifications

  • Registered Nurse licensed in South Dakota.
  • Minimum 10 years in long-term care.
  • Minimum 5 years as Director of Nursing in SNF.
  • Minimum 3 years Regional Nurse Consultant experience in LTC.
  • QAPI certification and Wound Care certification preferred.
  • Infection prevention experience preferred.
  • Experience applying State and Federal LTC regulations.
  • Knowledge of the long-term care survey process.
  • Strong written and verbal communication.
  • Willing to travel 90-100% across the division.

Responsibilities

  • Conduct onsite facility visits to assess operations, identify risks, and provide coaching.
  • Prepare facilities for surveys through mock surveys, regulatory reviews, and staff education.
  • Provide onsite support during regulatory surveys and assist leadership with response activities.
  • Develop, review, and monitor Plans of Correction (POCs).
  • Collaborate with facility leadership on corrective action plans and PIPs.
  • Analyze quality measures and regulatory reports to identify improvements.
  • Mentor and develop Directors of Nursing and other clinical leaders.
  • Assist with recruitment, onboarding, and succession planning for nursing leadership.
  • Review clinical documentation and policies to ensure regulatory compliance.
  • Support QAPI initiatives by identifying trends and monitoring outcomes.
  • Address admissions, infection prevention, wound care, falls, and behavioral health concerns.
  • Travel extensively to provide onsite consultation, education, and clinical support.

Regional Nurse Consultant South Dakota


Legacy Healthcare has an opportunity for a Regional Nurse Consultant in the State of South Dakota. This is a regional role and requires 90-100% extensive travel throughout the State of South Dakota, could include short notice travel, depending on surveys.

Position Summary:

The Regional Nurse Consultant provides clinical leadership, regulatory oversight, and operational support to multiple long-term care facilities. This position partners with Administrators, Directors of Nursing, and interdisciplinary teams to promote regulatory compliance, improve clinical quality outcomes, and ensure the delivery of safe, resident-centered care.

Qualifications/Skills
  • Registered Nurse currently licensed in the State of South Dakota

  • Minimum of ten (10) years of experience in a long-term care setting

  • Minimum of 5 years of experience as a Director of Nursing (DON) in a Skilled Nursing Facility

  • Minimum of 3 years of Regional Nurse Consultant (RNC) experience in long-term care. Multi state experience and divisional clinical oversite experience preferred

  • QAPI certification and Wound Care Certification preferred

  • Infection Prevention experience and Certification preferred

  • Experience applying State and Federal long-term care regulations

  • Knowledge of the long-term care survey process

  • Experience with policy development preferred

  • Deep understanding of healthcare legislation, adverse event regulations, and critical thinking

  • Strong communication, organizational, and writing skills

  • Ability to educate and mentor clinical and administrative staff

  • Proficient in regulatory documentation review and compliance monitoring

  • At least 90% extensive travel throughout the division

Key Responsibilities:
  • Conduct routine onsite facility visits to assess clinical operations, identify risks, and provide coaching and support.

  • Prepare facilities for annual, complaint, and revisit surveys through mock surveys, regulatory reviews, and staff education.

  • Provide onsite support during regulatory surveys and assist leadership with survey management and response activities.

  • Develop, review, and assist with writing Plans of Correction (POCs) and monitor implementation to ensure sustained compliance.

  • Collaborate with facility leadership to develop and implement corrective action plans and Performance Improvement Plans (PIPs).

  • Analyze quality measures, clinical indicators, survey trends, and regulatory reports to identify opportunities for improvement.

  • Mentor and develop Directors of Nursing and other clinical leaders through education, coaching, and leadership support.

  • Assist with recruitment, orientation, onboarding, and succession planning for Directors of Nursing and other key nursing leadership positions.

  • Review clinical documentation, policies, procedures, and facility practices to ensure compliance with regulatory standards and company expectations.

  • Assist facilities with reportable event investigations, determining reporting requirements, and reviewing reports submitted to regulatory agencies.

  • Support Quality Assurance and Performance Improvement (QAPI) initiatives by identifying trends and monitoring outcomes.

  • Assist facilities with clinical problem-solving related to admissions, staffing, infection prevention, wound care, falls, behavioral health, and other regulatory or quality concerns.

  • Travel extensively throughout the division to provide onsite consultation, education, and clinical support.
