Job summary

Outcome HC is seeking a Regional MDS Coordinator to ensure the highest quality of care for our residents. You will oversee the MDS process across multiple facilities, coordinate with clinical teams, and help maintain compliance with applicable regulations.

The ideal candidate has a strong clinical background in long-term care and excellent communication, organizational, and problem-solving skills. Training will be provided for the right candidate, with opportunities to improve clinical outcomes

  • Ability to work collaboratively as part of a healthcare team
  • Familiarity with MDS processes and regulations (training provided for the right candidate)

Responsibilities

  • Coordinate MDS process across multiple facilities within the region
  • Ensure accurate and timely completion of MDS assessments and maintain compliance with regulations
  • Collaborate with staff, physicians, and other healthcare professionals to improve quality of care
  • Coordinate MDS assessments and updates, ensuring accuracy and regulatory compliance
  • Develop and implement strategies to improve clinical outcomes and quality of care
  • Maintain records of MDS assessments and quality initiatives
  • Provide education and training to staff on MDS processes and clinical care standards
  • Participate in quality improvement initiatives and contribute to projects
  • Develop and maintain Case Mix Index (CMI) for the region
  • Work with finance team to maximize reimbursement

  • As a Regional MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) process across multiple facilities within our region
  • This includes ensuring accurate and timely completion of MDS assessments, coordinating with healthcare teams to address quality of care concerns, and maintaining compliance with all relevant regulations and standards
  • Your strong clinical background and excellent communication skills will enable you to effectively collaborate with staff, physicians, and other healthcare professionals to achieve our goals
  • Coordinate MDS assessments and updates, ensuring accuracy and compliance with regulations
  • Collaborate with healthcare teams to identify and address quality of care concerns
  • Develop and implement strategies to improve clinical outcomes and quality of care
  • Maintain accurate and up-to-date records of MDS assessments and quality care initiatives
  • Provide education and training to staff on MDS processes and clinical care standards
  • Participate in quality improvement initiatives and contribute to the development of quality improvement projects
  • Develop and Maintain Case Mix Index (CMI) for your region
