Job summary

Caring for Others in Warrenton, OR is seeking an experienced LPN Home Care Nurse to provide hands-on skilled nursing in clients' homes under RNs supervision.

This full-time role involves medication administration, patient assessments, caregiver training, accurate documentation, and collaboration with RNs and community providers to support safe, compassionate in-home care.

Qualifications

• Current LPN license in the state of Oregon required
• At least 4 years of management experience required
• Experience in home care, community health, or case management preferred
• Strong clinical assessment and case management skills
• Ability to train and mentor Caregivers effectively
• Excellent communication, organization, and documentation skills
• Working knowledge of electronic health records and care management systems (Epic, WellSky, AxisCare, or AlayaCare)
• Must have access to a reliable, insured vehicle and be willing to travel up to 100%

Responsibilities

• Travel across regional sites to provide direct, hands-on skilled nursing care to Clients in their homes under the direction of supervising RNs
• Administer medications and treatments in accordance with physician orders and nurse delegation protocols
• Monitor Client health status, documenting changes in condition and escalating concerns to the RN team promptly
• Support Client nursing evaluations and care plan updates alongside RN Case Managers
• Train and validate Caregiver competency in medication administration and nurse-delegated tasks
• Maintain accurate, compliant clinical documentation in line with company protocols and state regulations
• Assist with MAR review and medication administration audits across assigned Clients
• Collaborate with RNs, Caregivers, and community providers to support positive Client outcomes

POSITION SUMMARY

The LPN Home Care Nurse delivers hands-on clinical care and skilled nursing support to Clients in their homes, working under the direction of our RN team to ensure safe, compliant, and compassionate care. This role bridges skilled nursing standards with the personal, in-home setting - supporting Client health, medication administration, and Caregiver training across our service area.

ROLE RESPONSIBILITIES
