Job summary

Caring for Family of Companies is seeking an experienced LPN with strong management and case management skills to lead our in-home care team in Oregon. The role requires coordinating quality improvement, conducting nursing assessments, and ensuring accurate medication administration while traveling as needed.

You'll oversee MAR audits, train caregivers, and work with providers to maintain compliant documentation.

Qualifications

• Current LPN license in the state of Oregon required.
• At least 4 years of management experience required.
• Experience in home care, community health, or case management preferred.
• Strong clinical assessment and case management skills.
• Ability to train and mentor Caregivers effectively.
• Excellent communication, organization, and documentation skills.
• Knowledge of electronic health records and care management systems (Epic/WellSky/AxisCare/AlayaCare).
• Must have access to a reliable, insured vehicle and be willing to travel up to 100%.

Responsibilities

• Coordinate quarterly quality improvement meetings and analyze medication error reports to identify root causes and prevent recurrence.
• Conduct nursing assessments and create, audit, and reconcile MARs for accuracy and compliance.
• Perform 60- and 90-day medication and nursing monitoring visits, updating care plans and assessments as needed.
• Review and verify faxed medication orders, collaborating with healthcare providers to confirm accuracy.
• Manage changes in Client medication administration, ensuring seamless transitions and timely communication with Caregivers and staff.
• Lead training and competency evaluation for the medication administration program, including Caregiver redemonstrations and sign-off.
• Delegate nursing tasks to Caregiving staff, ensuring proper training, supervision, and adherence to care standards.
• Travel as needed to cover staff vacancies, perform Client evaluations, and maintain compliant nursing documentation.

Role Responsibilities
