Lutheran Services in Iowa (LSI) seeks a Contract Monitor to oversee the host home program in Eastern Iowa, ensuring service delivery and effective communication with interdisciplinary teams. The role involves travel across counties, with some work-from-home flexibility, and participation in an on-call rotation.

Qualified candidates have a HS Diploma/GED with five years of related experience, or an Associate’s/Bachelor’s degree with progressive related experience.

Qualifications

• Minimum Qualifications: HS Diploma/GED and five years of related experience; Associate’s Degree and three years; Bachelor’s Degree and one year.

Responsibilities

• Acting as the main contact for interdisciplinary team members and individuals served in the host home program.
• Work directly with assigned host homes to provide support, assistance, and instruction in areas which facilitate personal growth and independence.
• Facilitating and participating in interviews and evaluations related to the contracting process for independent providers.
