Matter of Care Home Health Services in New York is seeking an experienced Registered Nurse to mentor clinicians, oversee home-health operations, and elevate the quality of patient care for older adults in their homes.

Join an interdisciplinary team with leadership responsibilities, in-home assessments, wound care, and care coordination, while enjoying a hybrid schedule and weekday daytime hours. Requires NY RN license, BSN, and 3+ years of experience; infection-control certification is preferred.

Qualifications

• Active New York State RN license.
• BSN from an accredited nursing program.
• NYSED Infection Control Certification.
• Minimum 3 years of nursing experience.
• Valid NY Driver's License and reliable vehicle.
• Home Care, PACE, MLTC, Managed Care, or supervisory experience preferred.

Responsibilities

• Conduct joint supervisory visits with field nurses.
• In-home nursing visits for patients with complex medical needs, including assessments, treatment, and education.
• Mentor, coach, and develop nursing and paraprofessional staff.
• Audit clinical documentation for quality and regulatory compliance.
• Ensure evidence-based nursing standards are followed.
• Monitor care delivery and identify opportunities for improvement.
• Collaborate with physicians, care coordinators, and interdisciplinary teams.
• Provide wound care, medication administration, assessments, and patient education.
• Support Diagnostic & Treatment Center nursing operations as needed.

• $129,100.00 - $131,000.00 (5 – 9 years)
• $131,100.00 - $133,000.00 (10+ years)

Monday - Friday | 8:30 AM - 5:30 PM

What We're Looking For
