Job description

Oak Street Health®, a part of CVS Health, helps older adults stay healthier and live fuller lives through our 230 centers across 27 states. Our value-based care model helps us consistently deliver better patient experiences and outcomes.

As the nation’s leading health solutions company, we reach millions of Americans through our local presence, digital channels, and more than 300,000 purpose-driven colleagues — caring for people where, when, and how they choose in a way that is uniquely more connected, more convenient, and more compassionate. And we do it all with heart, each and every day.

Role Overview

As a Regional Director, Operations, you will play a critical role in supporting our Oak Street Health patients, providers and business by leading operational performance and population health execution across multiple centers within a geographic division. This role owns the delivery of key clinical, operational, patient experience, quality, utilization, and financial outcomes by ensuring that regional priorities are translated into disciplined execution across centers and care teams.

This leader reports directly to the Regional Vice President and works in close partnership with the Senior Medical Director, regional leaders, and central functional partners, including the central Population Health team. The role directly leads center operational leadership, including Practice Managers and other regional operational resources, and serves as a key cross-functional partner to clinical, service, outreach, and Treehouse teams.

Success in this role requires a leader who can drive results across a matrixed environment, build strong relationships quickly, and bring both strategic and operational rigor to a fast-paced, high-visibility setting. This individual will be accountable for improving the day-to-day performance of centers while also advancing Oak Street Health’s broader population health and value-based care priorities.

Core Responsibilities
Operational Leadership Across the Region
  • Lead operational oversight and performance across multiple centers within a geography
  • Ensure centers are operationally efficient and optimizing resources in service of patients
  • Drive strong execution in patient experience, patient engagement, schedule management, patient responsiveness, and center-level operational performance
  • Identify operational opportunities proactively and implement practical, sustainable solutions
  • Support center-level leaders in problem-solving, execution discipline, and continuous improvement
  • Share best practices and strengthen coordination across centers and markets
Population Health Ownership and Execution
  • Own execution and ongoing refinement of population health priorities across the division
  • Drive divisional performance across key population health domains, including transitions in care, pharmacy and medication adherence, HEDIS / Stars, accurate risk adjustment, preferred partner relationships, and patient engagement strategy
  • Ensure Oak Street Health’s clinical and business priorities are effectively implemented at the regional and center level
  • Maintain accountability for key quality, utilization, and financial performance measures tied to population health outcomes
  • Translate data and organizational priorities into focused regional action plans that improve quality, cost of care, and patient outcomes
Program Implementation and Cross-Functional Leadership
  • Lead implementation of centrally driven and regionally driven initiatives that improve operations and population health performance
  • Partner with central functional leaders to support strategy development, program design, workflow refinement, and execution feedback
  • Assess workflow impact across service, outreach, clinical, and operational teams to ensure effective implementation of new initiatives
  • Serve as a regional subject matter expert on operational and population health programs, priorities, and performance opportunities
  • Apply strong project management and change leadership practices to ensure initiatives are launched and sustained effectively
Leader Development and Regional Team Effectiveness
  • Lead, coach, and develop Practice Managers and other regional operational leaders with clear accountability for outcomes
  • Onboard, train, and support new leaders to build capability and consistency across the region
  • Conduct operational reviews focused on performance, quality, patient experience, and other key metrics
  • Foster a collaborative regional culture across operational and clinical teams
  • Support team development and strengthen consistency across centers in how care is delivered and operations are led
  • Model and reinforce Oak Street Health’s mission, culture, and standards throughout the region
Qualifications
  • 5+ years of leadership experience in healthcare, population health, clinical operations, or a related field
  • Multi-site leadership experience strongly preferred
  • Strong leadership and influencing skills, with the ability to work effectively across a matrixed organization
  • Demonstrated ability to build trust and credibility with a wide range of stakeholders, including providers, care teams, operational leaders, and central partners
  • Strong analytical and financial acumen, including experience using data to drive performance, develop business cases, and assess ROI
  • Excellent organizational skills, sound judgment, and ability to manage multiple priorities simultaneously
  • Strong communication skills and the ability to connect individual initiatives to broader organizational objectives
  • High degree of independence, reliability, and execution focus in a fast-paced environment
  • Quick learner who is comfortable operating in ambiguity and leading through change
  • Proficiency with Excel, Google applications, and project management tools
