Job summary

Longterm Health Management Services seeks a Director of Care to provide clinical leadership across a portfolio of approximately 10 skilled nursing facilities in Connecticut and Massachusetts. This hands-on role requires an RN or seasoned nursing leader who can drive consistent standards of care across sites.

The Director of Care collaborates with DONs, Administrators, and regional leadership to improve outcomes, regulatory compliance, staffing practices, and resident care.

Qualifications

• Current RN license in good standing; multi-state licensure is a plus.
• BSN preferred.
• Experience leading multiple SNFs or long-term care facilities.
• Prior DON/Regional Director of Nursing or similar leadership role preferred.
• Strong knowledge of CMS/state regulatory requirements and survey readiness.

Responsibilities

• Provide clinical oversight and leadership across multiple skilled nursing facilities.
• Partner with Directors of Nursing, Administrators, and facility leadership to maintain high standards of resident care.
• Conduct regular facility visits, clinical assessments, and operational reviews.
• Monitor quality measures, rehospitalizations, falls, infections, wounds, and medication management.
• Identify clinical risks and develop corrective action plans; support CMS/state surveys.
• Coach and mentor DONs and nursing leadership to strengthen clinical performance.
• Travel regularly between CT and MA facilities.
