Job summary

Continuing Healthcare Solutions is seeking a Regional Dietitian or Diet Tech to plan, implement, and provide nutrition services across northern facilities in our Niles and Lisbon region. Mileage reimbursement is included as you travel between CHS locations.

As an employee-owned ESOP company, you will be an owner and stakeholder in our growth, delivering high-quality nutrition care to residents and collaborating with an interdisciplinary team. OH licensure with RD is required.

Qualifications

  • Must hold Ohio licensure along with RD designation.
  • Bachelor's degree in Nutrition, Dietetics, or similar area of study.
  • Minimum of 1-year professional work experience in a Registered Dietitian role.

Responsibilities

  • Assesses patient’s knowledge of diet and kidney disease and provides education.
  • Provides patient-specific education to patient/family/c caregiver on nutrition and labs.
  • Completes nutritional assessment and participates in interdisciplinary assessments.
  • Develops written, individualized care plan with the team.
  • Calculates diet prescriptions per standard of practice.
  • Maintains documentation in medical records per federal/state regulations.
  • Provides ongoing nutrition assessment and counseling for renal patients.
  • Monitors and adjusts nutrition therapy as needed to reach goals.

Experience and Skills
Essential Functions
  • Assesses patient’s knowledge of diet and kidney disease andprovides education appropriate to patient’s learning needs regardingthe relationship of diet and kidney disease and the impact on thetreatment process.
  • Provides patient specific detailed education topatient/family/caregiver regarding nutritional status, adequacymeasures and results, fluid status, and all nutrition related labparameters pertaining to their disease process.
  • Completes nutritional assessment and participates in theinterdisciplinary comprehensive assessments.
  • Participates with the interdisciplinary team in the development ofa written, individualized comprehensive plan of care that specifiesthe services necessary to address the patient’s needs, in accordancewith established Federal and, where applicable, state licensure guidelines.
  • Calculates diet prescription according to standard of practice.
  • Completes progress notes on all chronic dialysis patients.Maintains complete and accurate documentation of all appropriateinformation in medical record according to established Federal andstate regulations.
  • Provides patient with ongoing nutrition assessment andoutcome-oriented nutrition counseling necessary to assist patient inachieving and sustaining an effective nutritional status.
  • Identifies malnourished patients as well as patients at risk formalnutrition and works collaboratively with interdisciplinary teamto identify appropriate interventions, resources or solutions.Reviews each patient’s albumin level and body weight at leastmonthly. Implements plan of care with involvement of patient/family/caregiver.
  • Monitors adherence and response to nutrition therapy, adjuststherapy as needed to achieve and sustain an effective nutritionalstatus, documents why the patient was unable to achieve the goalsand implements the plan of care to address issues impacting thepatient’s ability to achieve the goals.
  • Collaborates with the interdisciplinary team on management andrecommendations for mineral metabolism and renal bone disease,anemia and iron status, nutritional status, psychosocial status,vascular access, modality, rehabilitation status and sodium/volume control.
  • When appropriate, works with Social Worker to coordinate plan ofcare for patient education and identifies candidates to refer tophysician for nutritional supplements, as per disease management agreements.
  • Reviews nutrition related lab results:
  • Counsels patient and/or caregiver and formulates appropriate action.
  • Recommends treatment changes to the interdisciplinary team as appropriate.
  • Communicates with physician and/or facility staff regarding labresults as appropriate.
  • Communicates lab results and recommended treatment changes toproviders of patient care in nursing facilities (i.e. hospitals,short term and long term care facilities).
  • Participates in pertinent staff meetings (i.e. general staff,quality improvement), Care plan meetings, and others as applicable.
  • Consults with Medical Director regarding nutrition concerns as needed.
  • Maintains and improves knowledge and skills for a competent andinnovative practice.
  • Maintains dietetic registration and continuing education hours asspecified by American Dietetic Association and state licensureregulations where applicable.
  • Collaborates with interdisciplinary team, ensuring that all workareas are safe and clean.
  • Order and monitors supplements.
  • Other nutrition management duties as assigned.
  • Ability to work flexible schedules, including traveling tomultiple facilities within the region when needed.
Education Requirements

Must hold Ohio licensure along with RD designation as required by CHS.

Bachelor's degree in Nutrition, Dietetics, or similar area of study.
