Job summary

AdviniaCare is seeking a Regional Clinical Liaison (LPN or RN) for Rhode Island. This full-time, field-based role connects hospitals, hospice agencies, and community organizations with our facilities to help patients and families find the right program.

You'll travel within Rhode Island, meet patients where they are, and conduct on-site evaluations while collaborating with Admissions and nursing staff to ensure smooth care transitions.

Qualifications

• Active Rhode Island LPN or RN license.
• Experience as clinical liaison, admissions, or referral experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Respond promptly to referrals from hospitals, hospice agencies, and community social service partners.
• Monitor referral platforms for qualified admissions in your coverage area.
• Conduct on-site clinical evaluations to determine program fit for patients.
• Share completed evaluations with Admissions Director and provide a verbal overview of patient condition.
• Educate patients and families about programs, answer questions, arrange facility tours.
• Screen local hospital referrals for AdviniaCare facilities outside Rhode Island as needed.
• Build and strengthen relationships with referral sources, community contacts, and facility teams.
• Collaborate with Department Heads and nursing and rehab staff.
• Track referrals by hospital, participate in census calls, and provide reports.
• Support marketing and outreach efforts.

Are you a nurse who loves connecting people with the right care? As AdviniaCare's Regional Clinical Liaison for Rhode Island, you'll be the bridge between hospitals, hospice agencies, community organizations, and our facilities. You'll meet patients where they are, assess their needs, and help them and their families find a program where they'll thrive.

This role is ideal for a clinically sharp, people-focused nurse who enjoys working independently, building relationships, and spending the day out in the community rather than on a unit.

What You'll Do

• Respond promptly to referrals from hospitals, hospice agencies, and community social service partners
• Monitor electronic referral platforms for qualified admissions in your coverage area
• Conduct on-site clinical evaluations to determine whether a patient is a good fit for our programs
• Share completed evaluations with the Admissions Director, along with a verbal overview of the patient's condition
• Educate patients and families about our programs, answer their questions, and arrange facility tours with our Admissions Coordinator
• Occasionally screen local hospital referrals for AdviniaCare facilities outside Rhode Island
• Build and strengthen relationships with referral sources, community contacts, and facility teams
• Collaborate with Department Heads and nursing and rehab staff
• Track referrals by hospital, participate in daily and weekly census calls, and provide reports as needed
• Support marketing and outreach efforts

What You Bring

• Active Rhode Island LPN or RN license (required)
• Previous clinical liaison, admissions, or referral experience strongly preferred
• Strong communication and interpersonal skills, with the ability to make families feel at ease
