Job summary

CenterLight Health System in New York is seeking a Regional Clinical Lead - Nurse Practitioner to provide advanced practice and clinical leadership across TeamCare sites within the assigned region.

The NP will perform histories, exams, chronic-disease management, education, and timely documentation in the EHR, while supporting same-day access, transitions of care, and quality-improvement initiatives in a dynamic primary-care network.

JOB PURPOSE

The Regional Clinical Lead - Nurse Practitioner, works in collaboration with TeamCare primary care physicians and clinical leadership to ensure appropriate oversight, coordination, and continuity of participant care across an assigned region. The role provides advanced practice and clinical leadership coverage at designated sites based on operational and clinical needs. Core responsibilities include gathering detailed histories; performing physical examinations and relevant diagnostic testing; diagnosing and treating common medical conditions within the nurse practitioner's scope of practice; managing chronic illnesses and tailoring treatment plans; educating participants and their families about disease prevention, health promotion, and slowing the progression of chronic illness; making medically appropriate specialist referrals; maintaining accurate and complete documentation in the electronic health record (Athena); adhering to professional standards of conduct; participating in quality-improvement initiatives; and collaborating effectively with healthcare professionals, colleagues, participants, and their families.

JOB RESPONSIBILITIES

Provide Nurse Practitioner and clinical leadership coverage across designated TeamCare sites within the assigned region, serving a diverse adult participant population. Travel regularly among assigned sites and adjust the primary work location based on operational, staffing, and participant-care needs. Support continuity of clinical operations across the region, including coverage for vacancies, leaves, scheduling gaps, increased participant volume, and other emerging needs. In the absence of, or at the direction of, the Medical Director, represent or support the Medical Director function at the site being covered, within the scope of the role and delegated authority. Collaborate with Medical Directors, Associate Clinical Directors, site leadership, and interdisciplinary teams to promote consistent clinical practices and effective handoffs across assigned locations. Identify site-level clinical or operational concerns, communicate them promptly to the appropriate leader, and assist with implementing corrective or improvement actions. Perform annual wellness assessments. Educate participants to ensure they understand their medical complexities and comorbidities. Perform routine quarterly assessments for participants with chronic medical illnesses. Provide acute care services to participants and facilitate same-day access. Assess and manage participants with acute changes in condition in a timely manner through face‑to‑face or telemedicine visits. Conduct post‑hospitalization visits within 7-14 days of discharge, including medication reconciliation, treatment‑plan development, and coordination with specialists to support a timely and effective transition of care. Develop a comprehensive and accurate diagnostic list of current and past medical conditions using clinical knowledge, judgment, and assessment findings. Ensure all documentation is complete and accurate, includes specific diagnosis codes, and is entered in the electronic health record on the same day as the visit. Review participants' current symptoms and exacerbations of previously controlled conditions; identify active diagnoses and chronic conditions requiring medical management, treatment, or intervention. Provide clinical oversight of care‑plan development and delivery by each discipline for community PCP participants; ensure care plans are individualized and that interventions and outcomes are appropriately followed and supervised by the practitioner. Practice only within the individual's authorized scope of practice. Meet the standardized competencies established by TeamCare for this position before working independently. Share relevant clinical and operational information with the receiving site or provider to support safe transitions and continuity of care. Perform other duties as assigned.

QUALIFICATIONS

Education: Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.

Experience: Minimum of two (2) years of clinical practice in a primary care setting, home care, care management, or a related field required. Minimum of one (1) year of experience working with an adult population. An individual with less than one (1) year of experience who meets all other requirements must receive appropriate training from TeamCare.

Effective oral and written communication and interpersonal skills required. High‑level analytical and research skills. Ability to manage multiple projects and meet project deadlines. Basic computer proficiency, including Microsoft Office.

License: Current license to practice as a Nurse Practitioner in New York State required. Current license to practice as a Registered Nurse in New York State required. Current DEA registration required. AANP or ANCC Board Certification required.

Additional Requirements: Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function. Be medically cleared for communicable diseases and have all immunizations up‑to‑date before engaging in direct participant contact. Must have a Medicaid Provider ID and a Medicare Provider ID in good standing.

Language: Bilingual preferred.

Physical Requirement: Individuals must be able to sustain certain physical requirements essential to the job. This includes, but is not limited to: Standing – Duration of up to 6 hours a day. Sitting/Stationary positions – Sedentary position in duration of up to 6‑8 hours a day for consecutive hours/periods. Lifting/Push/Pull – Up to 50 pounds of equipment, baggage, supplies, and ability to lift patients safely and using OSHA guidelines, etc. Bending/Squatting – Must be able to safely bend or squat to care for patients, use medical supplies, etc. Stairs/Steps/Walking/Climbing – Must be able to safely maneuver stairs, climb up/down, and walk to access work areas. The position requires the individual to be able to travel and walk between sites/locations and work areas throughout the day. Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools (i.e., typing, use of medical supplies, equipment, etc.) Sight/Visual Requirements – Must be able to visually assess patients, read orders, type, and write documentation, etc. with accuracy. Audio Hearing and Motor Skills (language) Requirements – Must be able to listen attentively and document information from patients, community members, providers, etc., and intake information through audio processing with accuracy. In addition, must be able to speak comfortably and clearly with language motor skills for customers to understand the individual. Cognitive Ability – Must be able to demonstrate good decision‑making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.
