Job summary

Healthbridge Staffing is seeking a Regional Case Manager to support multiple skilled nursing facilities across the West Coast (OR/WA). The role focuses on admissions guidance, care coordination, and discharge planning to optimize transitions and outcomes.

You will partner with hospital systems, managed care organizations, and interdisciplinary teams to improve reimbursement, census, and operational performance while traveling regionally as needed.

Qualifications

  • Current RN, LPN, LSW, PT, OT, or ST license preferred.
  • MDS certification and/or post-acute case management experience strongly preferred.
  • Minimum of 3 years of experience in skilled nursing, post-acute care, hospital discharge planning, care coordination, or case management.
  • Strong understanding of Medicare & Medicaid, Managed Care, Insurance Authorizations.
  • Care Transitions, Discharge Planning, Skilled Nursing Operations, Clinical Documentation.
  • Exceptional communication, relationship-building, and organizational skills.
  • Proficiency with EMR and Microsoft Office.
  • Ability to travel up to 50%.

Responsibilities

  • Provide regional case management support for multiple SNFs across OR/WA region.
  • Collaborate with interdisciplinary teams to coordinate admissions, transfers, and discharges.
  • Review referrals to ensure placement aligns with facility capabilities.
  • Support facilities in reducing readmissions and improving resident outcomes.
  • Assist with complex discharge planning and care coordination initiatives.
  • Partner with hospitals, physicians, managed care, and insurers for timely admissions and authorizations.
  • Monitor payer trends and optimize reimbursement and operations.
  • Develop relationships with hospital partners and referral sources.
  • Travel regionally to provide onsite support, education, and guidance.
  • Support census growth and improve referral conversion and admission processes.

Position Summary

The Regional Case Manager serves as a clinical resource for assigned skilled nursing facilities by providing guidance on admissions, care coordination, managed care processes, discharge planning, and census development. This role works closely with facility leadership to improve operational performance while ensuring residents receive the highest quality of care.

Key Responsibilities
Care Coordination & Case Management
  • Provide regional case management support for multiple skilled nursing facilities throughout the Midwest Region.
  • Collaborate with interdisciplinary teams to coordinate admissions, transfers, and discharges while ensuring seamless transitions of care.
  • Review clinical referrals to ensure appropriate placement and alignment with facility capabilities.
  • Support facilities in reducing avoidable hospital readmissions and improving resident outcomes.
  • Assist with complex discharge planning and care coordination initiatives.
Managed Care & Clinical Review
  • Partner with hospitals, physicians, managed care organizations, and insurance providers to facilitate timely admissions and authorizations.
  • Review clinical documentation to support medical necessity and reimbursement.
  • Assist facilities with payer communications, authorization processes, and insurance requirements.
  • Monitor Medicare, Medicaid, and managed care trends while identifying opportunities to improve reimbursement and operational performance.
Regional Operations Support
  • Develop and maintain strong relationships with hospital partners, referral sources, and community healthcare providers.
  • Support census growth initiatives by improving referral conversion and admission processes.
  • Assist facility leadership with case management best practices and regulatory compliance.
  • Travel throughout the Midwest Region to provide onsite support, education, and operational guidance.
  • Collaborate with regional leadership on strategic initiatives designed to improve quality outcomes and operational success.
Qualifications
