Job summary

CHA is hiring a Region III ROOTS Hub Program Director to provide strategic leadership and oversight across six counties. The role serves as the primary regional liaison with health departments, hospitals, HRAs, and partners to translate priorities into community-driven strategies.

The candidate will manage performance monitoring, reporting, and cross-functional coordination, ensuring implementation aligns with ROOTS Hub goals and regional needs.

Qualifications

  • Bachelor's degree in public health, health administration, social work, community development, business, or related field.
  • Seven (7) or more years of progressively responsible experience in public health, health care, community development, program management, care coordination, rural health, or a related field.
  • Master's degree in an applicable field is highly desired.

Responsibilities

  • Serve as a trusted ambassador and primary point of contact for Region III ROOTS Hub activities and represent the Hub at regional, county, community, and stakeholder meetings
  • Build effective relationships across the six counties, align implementation activities, and identify opportunities for cross-county collaboration and resource sharing
  • Ongoing program assessment through monitoring performance of deliverables, milestones, identification of emerging opportunities and risks while promoting successful local strategies
  • Review and submit quarterly reports, contract compliance, financial documentation, and annual reports
  • Support development and implementation of action plans, including concrete next steps, responsible parties, timelines, and measures of success
  • Facilitate care pathway, referral, workforce, and service coordination discussions while monitoring progress and opportunities to accelerate activities
  • Assess partner readiness to participate in ROOTS Hub activities and help organizations understand expectations, roles, responsibilities, and available resources

Salary Range: $90,000.00 To $110,000.00 Annually

Hours: 8:00 - 5:00 Monday - Friday, 40 hours per week

Funding period through June 30, 2028

Must have ability to travel throughout six-county Region III service area

The Region III ROOTS Hub Program Director provides strategic leadership and operational oversight for the Region III ROOTS Hub across its six-county service area. Serving as the primary regional liaison, the Program Director fosters collaboration among local health departments, health care providers, hospitals and health systems, community-based organizations, local governments, educational and workforce partners, residents, Vaya Health, Appalachian State University, AHEC, and other ROOTS Hub stakeholders. The Program Director works closely with Catawba County Public Health, the region's second public health anchor for Medicaid Region III, to advance shared goals and coordinate implementation efforts. This position is responsible for translating regional priorities, community needs, and assessment findings into practical, community-driven strategies that support the ROOTS Hub mission.

  • Knowledge of rural health, community-based systems, public health, health care systems, and community development

  • Must Possess strong relationship-building skills and demonstrate exceptional organizational abilities
  • Excellent communication skills, both written and verbal, are essential
  • Ability to to work with a diverse community and organizational partners as well as lead collaborative initiatives
  • Possess ability to successfully manage competing priorities, tight deadlines, and cross-functional stakeholder relationships
