Job summary

Rural Health Care Inc Dba Aza Health in Florida is seeking a Referral Specialist to assist patients by referring them to outside medical specialists or agencies at the request of AH attending physicians. This is a full-time position with a Monday-Thursday 8:00–6:30 schedule.

Key duties include contacting outside physicians, coordinating appointment locations and times, and maintaining referral documentation and logs to support patient care.

Responsibilities

• Contact outside physicians or agencies by phone to arrange appointments and exchange patient information.
• Coordinate appointment locations and times with patients.
• Maintain a list of agencies for patients needing financial aid or other assistance.
• Prepare referral logs and reports documenting patient referrals.
• Screen applicants for county prescription program.
• Obtain and document HMO authorizations in patient records.

The Referral Specialist is responsible for assisting patients by referring them to an outside medical specialists or agencies at the request of AH attending physicians.

POSITION RESPONSIBILITIES

• Contacts outside physicians or agencies by telephone, arranges patient appointments and coordinates the exchange of patient information between AH providers and the outside physicians or agencies.
• Works closely with patients to arrange and coordinate appointment locations and appointment times.
• Maintains list of agencies that can be contacted by patients requiring financial aid or other assistance.
• Prepares referral logs, weekly reports, monthly reports, documenting patient account and other necessary referral documentation.
• Screens applicants for the Putnam County Health Care Authority prescription program.
• Obtains authorizations from HMO’s and documents authorizations in patient medical records.
