Responsibilities

• Screen referrals using the designated screening tool and rescreening process.
• Manage statewide referral screening assessment process.
• Coordinate admission paperwork for timely completion and accuracy.
• Provide statistics tracking referrals and conversion rates.
• Communicate assessment findings to Administrator and ensure timely referral assessments.
• Maintain an active waiting list file and reconcile with State of Georgia for slot assignment.
• Submit activity reports as requested by Vice President.
• Contact referral sources to track members referred to the program.
• Understand referral process and COURCE policies and procedures.

Description

JOB PURPOSE:

Screen appropriate referrals and coordinates admission process. Coordinates services, coordinates services with providers, and billing reconciliation. Provides outreach to community referral sources. This position will be hybrid or remote.

KEY RESPONSIBILITIES:

• Responsible for using the designated screening tool to complete referral screening and rescreening process.
• Primary responsibility to manage and oversee the referral screening assessment process statewide.
• Coordinates admission paperwork to promote timely completion and accuracy.
• Provides statistical data that tracks number of referrals and conversion rates.
• Provides communication of assessment findings to Administrator and makes sure referral assessments are completed timely and included on referral statistic log for review.
• Maintain active waiting list file.
• Reconciles wait list and submits file to State of Georgia for slot assignment.
• Completes and submits activity reports as requested by Vice President.
• Contacts referral sources as needed to track members referred to the program.
• Complete understanding of the referral process.
• Knowledge and understanding of COURCE policies and procedures including the: MDS HC assessment process.
• Excellent communication and organizations skills. Proficient in Excel, Word, and email.

KNOWLEDGE, SKILLS, ABILITIES:

• Ability to multi-task
• Microsoft office suite knowledge

MINIMUM EDUCATION REQUIRED:

Nursing Degree from and accredited college or University, or is a graduate of an accredited/approved RN or LPN program

MINIMUM EXPERIENCE REQUIRED:

Five years nursing experience

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:

Current and unrestricted RN or LPN licensure in sate of practice

CPR Certified

Must have valid driver’s license and auto coverage

ADDITIONAL QUALIFICAITONS:

SOURCE, Home Health, Community Services, or Case Management experience preferable.
