Job summary

Enhabit Home Health & Hospice is seeking a regional coordinator to oversee patient referrals and support branch staff in transitioning to home health services.

The role requires knowledge of Medicare guidelines for home health, a high school diploma, and valid driver's license; in-office attendance is essential and remote work is not allowed. Strong communication and Office skills are expected, with preference for Homecare Homebase experience.

Qualifications

• High school diploma or equivalent required.
• At least one year of home health experience.
• Knowledge of Medicare guidelines for home health.
• Valid driver's license in the state of residence.
• Auto liability insurance as required by law.
• Graduate of an approved school of clinical education, nursing, or therapy preferred.
• CPR certification and valid state license as applicable.
• Strong communication and active listening skills.
• Proficient with Microsoft Office (Word, Excel).
• In-office attendance required; remote work not permitted.

Responsibilities

• Coordinate patient referrals regionally; act as a resource for referral sources and ensure smooth transition to home health services.
• Demonstrate knowledge of regulatory guidelines governing home health admissions and represent the branch with referral sources.

Responsibilities

• Responsible for regional coordination of patient referrals. Acts as a resource for referral sources and works closely with branch staff to ensure adequate coordination and transition to home health services.
• Demonstrate extensive and current knowledge of regulatory guidelines governing home health admissions and represent the branch in activities involving professional contacts with referral sources.

Qualifications

• Minimum of a high school diploma or equivalent.
• At least one year of experience in home health.
• Demonstrated knowledge of Medicare guidelines governing home health.
• Valid state driver’s license in the current state of residence.
• Valid automobile liability insurance as required by law.
• Graduate of an approved school of clinical education, nursing, or therapy is highly preferred.
• Clinical license holders must maintain a valid license in the state providing care, per state requirements, and CPR certification.
• Previous experience using Homecare Homebase (HCHB) is preferred.
• Strong oral and written communication skills, fluency in English, and active listening.
• Intermediate technology skills, especially Microsoft Office (Word, Excel).
• In‑office attendance essential for full‑time remote‑designated positions; remote work not permitted except as specified.
