Allina Health is seeking a Care Navigation and Outreach professional to guide and connect patients to appropriate care and resources. This role involves intake, enrollment, and documentation in the electronic medical record to support coordinated care.

Qualified candidates bring 0–2 years in health care or customer service and proficiency with Microsoft Office tools. The position offers a day shift and a remote hybrid schedule (80% remote, 20% in-person) in Minneapolis.

Qualifications

• Proficient in computer technology and Microsoft Office applications including Teams, Word, Excel and SharePoint
• Ability to communicate clearly with patients and families

Responsibilities

• Coordinate referrals on behalf of patients.
• Answer calls from internal and external customers and guide patients to resources.
• Verify patient eligibility for referred programs and inform about scope and limitations.
• Document in the electronic medical record and ensure data accuracy.
• Schedule appointments inside and outside the system as ordered.
• Perform intake and enrollment calls and data entry for programs.
• Transition patients to the next step of care with complete information.
• Communicate with patients, families, providers and care team to maintain coordination of care.
• Assist with understanding health plan eligibility and questions as needed.

Key Position Details

Job Description

Serves as a central person to guide and connect patients to the right care at the right time. Assists patient in being connected to the right resource, verifying patient eligibility and health plan benefits, performing intake and enrollment functions and utilizes the electronic medical record to document, communicate and facilitate care for patients.

Principle Responsibilities

• Coordinates referrals on behalf of patients.
• Receives calls from internal and external customers and provides on touch resource to guide patients to appropriate resources.
• Ensures patient eligibility criteria for referred program.
• Informs patients and families of the scope and limitations of a service and payer source.
• Documents in the electronic medical record.
• May schedules appointments, inside and outside of the system, as ordered.
• Performs intake, enrollment calls and data entry for designated programs.
• Transitions patient safely to next step of care by collecting information needed and documents in electronic medical record.
• Communicates with patients, families, providers and care team to maintain coordination of care.
• May verify health plan eligibility.
• May act as a resources for patients, providers and staff as in pertains to specific health plans and other questions.
• Other duties as assigned.

Required Qualifications

• Must be 18 years of age with education and/or experience needed to meet required functional competencies as listed on the job description
• 0 to 2 years of related experience in health care or customer service
• Proficient in the use of computer technology and Microsoft Office applications, including Microsoft Teams, Word, Excel and SharePoint

Preferred Qualifications

• High school diploma or GED
• Associate’s or Vocational degree

Physical Demands

• Sedentary:
  Lifting weight up to 10 lbs. occasionally, negligible weight frequently

Pay Range

Pay Range: $20.79 to $28.31 per hour

The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
