Job description

About Us

At Greenbrook Medical, we believe seniors deserve more from the healthcare system—more time, more care, more coordination, and more heart. We provide high-touch, relationship-based primary care to seniors, built around one simple idea: deliver the kind of care we’d want for our own parents.

Founded by two brothers inspired by their father’s pioneering work in Medicare Advantage, Greenbrook is deeply personal and proudly modern. We quarterback our patients through their healthcare journey, making sure they’re never alone in a complex system. Our business model is designed around patient outcomes, not volume—so we only succeed when our patients thrive.

With roots in Tampa Bay and a partnership with Tampa General Hospital, we’re growing thoughtfully to bring our model to more communities. Our team is the heart of it all: mission-driven, values-oriented, and relentlessly committed to taking the best care of our patients.

About The Role

We are seeking a detail-oriented and organized Referral Coordinator to join our team. The Referral Coordinator will be responsible for managing the referral process from start to finish, ensuring timely communication between patients, providers, and insurance companies. This role will also support the clinical team by fostering meaningful relationships with patients and their families and caring for our patients in a hands-on and personalized way. Reporting to the Center Manager, this role offers the opportunity to take on more responsibility as we grow.

Key Responsibilities

Referrals/authorizations Management:

  • Manage the referral process by sending referrals and submitting authorizations, ensuring all information is accurate and complete before submission.
  • Track referral progress, including approvals, denials, and appointments, and assist in coordinating medical records and documentation as needed.
  • Manage patient records and ensure accurate documentation in the EMR system.
  • Coordinate scheduling and follow-up appointments to optimize patient care.
  • Respond to patient and inbound provider inquiries promptly and professionally, offering concierge-level support.

Front Office/MA Support (when Needed), Including:

  • Verify patient eligibility and insurance coverage, and act as the main point of contact for patients, providers, and insurance companies regarding referral status and concerns.
  • Schedule appointments, provide clear instructions to patients, and follow up with medical offices, insurance companies, and patients to ensure timely handling of referrals and record collection.
  • Partner with providers and other staff to create seamless patient experiences.
  • Contribute to care team discussions to enhance patient outcomes.
  • Maintain a patient-centered mindset while upholding clinic standards of care.
  • Be an extension of our MA team.
About You
