Job summary

Community Health and Wellness, LLC seeks Certified Therapeutic Recreation Specialists to provide home and community Recreation Therapy for children and adolescents with disabilities across Texas. Contractors must travel to consumer homes and facilities in the San Antonio area.

Responsibilities include creating tailored goals, documenting sessions, and coordinating with the care team. Valid CTRS/TRS/TXC certification, Bachelor's degree in therapy recreation, CPR/First Aid, and proper licensing

Qualifications

• Bachelor's degree in Therapeutic Recreation or related field is required.
• CPR/AED/FIRST AID certification is required.
• CTRS or TRS/TXC certification is required.
• Social Security Card and Driver's License are required.
• Auto Insurance (100/300/100) and Professional Liability Insurance are required.
• Background and reference checks will be conducted.

Responsibilities

• Provide one-on-one home and community Recreation Therapy to children and adolescents with disabilities.
• Create tailored goals to meet individual client needs.
• Document on each session and track SMART goals.
• Communicate with health team members and service providers.
• Submit weekly invoicing and related paperwork.
• Travel to clients' homes; transport clients during visits.
• Implement patient care plans through home and community activities.

Community Health and Wellness, LLC is looking for professional Certified Therapeutic Recreation Specialists to provide top notch services for individuals with disabilities throughout Texas. We have current openings in San Antonio, TX. Contractors must have reliable transportation and a willingness to travel to consumer homes and facilities.

Job Description:

• Provide one-on-one home and community Recreation Therapy to children and adolescents with intellectual disabilities, physical disabilities, and behavioral/mental health diagnoses
• Create tailored goals to meet the needs of individual clients
• Document on each session
• Communicate with other team members and service providers
• Turn in timely documentation including weekly invoicing, progress notes, and related paperwork

Essential Duties and Responsibilities

• Must be able to travel to current or prospective clients' homes
• Must be able and willing to transport client(s) during client visits
• Must have reliable transportation, a valid driver's license and agency-required auto liability insurance (100k/300k Bodily Injury)
• Must be able to communicate with health care professionals, clients, families, and the general public through verbal and written communication
• Must have access to a private personal laptop or desktop computer for HIPAA compliance
• Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader
• Implement patient care plans through home and community goal-related activities
• Able to work independently, manage one's own time and the time of client(s)

All therapists must have the following:

• Resume
• Minimum of Bachelors Level Degree required in one of the following: Therapeutic Recreation, Recreation Administration with emphasis in Therapeutic Recreation, Leisure Studies with emphasis in Therapeutic Recreation, and/or related degree to Therapeutic Recreation
• CPR/AED/FIRST AID Certification (American Red Cross or American Heart Association only)
• CTRS or TRS/TXC certified
• Social Security Card
• Driver's License
• Auto Insurance (with 100/300/100 coverage minimum)
• Professional Liability Insurance (with $1 million/$3 million coverage limit)

Hourly Rate is based on education/experience.
