Job summary

Harboroaks is seeking a behavioral health program facilitator responsible for delivering and implementing recovery programs within a facility. You will provide patient care, coordinate activities, and collaborate with the treatment team to support treatment goals.

Responsibilities include group co-leadership, monitoring patient well-being, and ensuring accurate documentation. A positive and empathetic approach to residents is essential to maintaining safety and quality care.

Qualifications

• High School diploma or equivalent required.
• Bachelor's degree in Human Services, Social Work or related field preferred.
• One or more years previous experience in behavioral health with population of the facility preferred.

Responsibilities

• Provide patient care to include coordinating activities, providing companionship and assisting with basic daily needs (dressing, bathing, eating).
• Collaborate with treatment team members to assist with patient treatment and/or care goals.
• May serve as a co-lead for groups.
• Monitor the daily well-being of patients and identify need for social intervention.
• Ensure all documentation regarding the program, such as care, treatment or incidents, is completed in a clear, concise, professional and completed in a timely manner.
• Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
