Evergreen Health in Buffalo, NY is seeking a Recovery Peer Advocate to support recovery goals for patients with substance use and/or mental health concerns. This full-time role involves outreach, advocacy, mentoring, coordinating with treatment options, and non-clinical crisis support.

The ideal candidate has a high school diploma or GED, CRPA certification or willingness to complete CRPA-P within 60–90 days, and experience with diverse populations. Familiarity with Medent EMR is preferred.

• CRPA certification or willingness to complete CRPA-P within 60–90 days.
• Familiar with targeted communities and diverse patient populations.
• Experience with EMR software and medical terminology preferred.

Responsabilités

• Engages with individuals onsite and in the community to consider treatment.
• Provides outreach, advocacy, mentoring and recovery support services.
• Encourages patients to attend treatment or healthcare services and navigates options.
• Provides non-clinical crisis support.
• Develops recovery plans with patients and completes required documentation.
