Job summary

LifeWorks NW in Beaverton is seeking a Certified Recovery Mentor to provide case management, counseling referrals, family support and coordinated services to clients in treatment. You will engage clients in recovery, help with housing and employment issues, and work closely with therapists and community partners.

The role requires CRM certification, addiction experience, confidentiality, and willingness to work some evenings or weekends at the Beaverton site.

Qualifications

• High school diploma/GED required.
• Current Certified Recovery Mentor (CRM) with MHACBO.
• Extensive experience with addiction issues and related language/values.
• Ability to maintain confidentiality and appropriate boundaries.
• Experience navigating the social service network.
• Willingness to work some evenings and/or weekends.

Responsibilities

• Screen and select clients to participate in the mentoring program.
• Develop the service plan for all clients including a service reporting plan on each referral.
• Within five working days after intake, meet with the client and family to develop a case management plan.
• Maintain daily contact with clients for up to five weeks after release to support recovery, housing, employment and family issues.
• Coordinate with the client’s therapist, collateral providers, and community corrections staff.
• Determine when to terminate services if no longer needed or if the client does not respond to the mentor model.
• Document and maintain client records in the electronic health system within 24 hours of meetings.
• Travel between sites or in the community to assist clients.
• Participate in trainings and maintain cultural competency.
• Provide services in a culturally appropriate manner.

The Certified Recovery Mentorprovides case management offering a combination of referral linkage, individual counseling, family support, group counseling and case management services to assist client's successful engagement in treatment and the family's ability to support the client's continued treatment involvement. May work in a residential or outpatient setting.

Location:This position is based out of LifeWorks NW's Beaverton Site.

Pay/Benefits:$22.18-$25.50 per hour, plus benefits .

Salary Placement Notice:
The posted salary range reflects the compensation potential for this role. Most candidates will be placed between the entry level and midpoint of the range, depending on their experience and qualifications. However, more seasoned professionals with extensive exact or similar experience may be considered for placement toward the higher end of the range. Final salary offers are made in alignment with internal compensation processes in accordance with Oregon Pay Equity laws.

ESSENTIAL RESPONSIBILITIES

• Screens and selects clients to participate in the mentoring program.
• Develop the service plan for all clients including a service reporting plan on each referral.
• Within the first five working days following intake into services, the client and (if appropriate) family members meet with the Recovery Mentor and the Primary Counselor to develop a case management plan.
• The Recovery Mentor should have daily contact with clients for up to the first five weeks after release helping with engagement in recovery support, housing and employment issues, family adjustment issues, legal, housing, medical and benefit issues. Frequency of contact should gradually decrease to the point of twice monthly or monthly contact by the ninetieth day after release.
• Responsible for assisting the family in the initial implementation of the plan, maintaining regular communications with the client's primary therapist, collateral providers, community corrections staff, and linking the client system to needed services within the community.
• Determines when to terminate clients from the program because services are no longer necessary, or the client is not responding to the mentor model.
• Document and maintain client records in electronic healthcare system within 24 hours of meeting with client to ensure compliance with agency and contract guidelines and confidentiality.
• Meets regularly with staff from social service agencies to update them on LifeWorks NW services identifying unmet services/needs LifeWorks NW may be able to address; review and update referral and case coordination procedures as needed; obtain referral requests and work with referral sources to remove any barriers to treatment; report back if clients fails to attend; obtain relevant case information from the referral for the clinician who will be doing the client's intake assessment; work with referral source and client to facilitate the family's involvement in the assessment and the treatment planning process.
• Attend informational and clinical meetings which may include all-agency meetings, team meetings, consultation meetings with psychiatrist, staffing, intra-agency trainings, etc. as required.
• All services must be provided in a culturally appropriate manner.
• Meets standards for documentation and work accuracy as defined in the Performance Evaluation including timely completion of progress notes, assessments, treatment plans, and authorization requests.
• Meets standards aligned with Key Results and as defined in the Performance Evaluation for direct service, implementation and utilization of the clinical model, quality of clinical practice and teamwork.
• Travels between sites or in the community driving clients when needed.
• Continually increases cultural sensitivity, awareness and competency. Actively participates in a minimum of four hours of training related to cultural effectiveness in working with diverse populations.
• Participate in a culture of feedback at the agency by eliciting feedback to promote professional growth and assess work effectiveness.

REQUIREMENTS

• High school diploma/GED.
• Must have a current Certified recovery Mentor Certificate (CRM) with MHACBO.
• Substantial experience with issues involving addiction including the language, values, norms and other attributes of the subculture; the subtleties and linguistic nuances of the differences between honest dialogue and statements made for effect.
• The ability to maintain confidentiality and appropriate boundaries.
• Extensive knowledge of how to efficiently navigate the social service network from the bottom up.
• Must be able to accompany clients throughout their environment.
• Must have knowledge of the legal/corrections system.
• Demonstrates effective communication skills and maintains a cooperative working relationship with others in a culturally diverse environment.
• Requires ability to work some evenings and/or weekends
• Requires valid driver's license, proof of current automobile insurance coverage, verification of safe driving record from DMV, and ability to meet LWNW driving requirements.
• Ability to perform essential functions of job without creating a direct threat to the safety of self or others.
