Job summary

COORDINATED CARE service inc. seeks a Vocational Rehabilitation Counselor to deliver vocational rehab, employment counseling, job readiness, and placement services to individuals with substance use disorders in the Finger Lakes region.

The role collaborates with employers, education providers, and community partners to expand opportunities and reduce barriers to employment, with documentation and compliance responsibilities throughout the region.

Qualifications

• Master’s degree in Vocational Rehabilitation, Counseling, Social Work, Psychology, Human Services, or related field plus 1 year of experience; or Bachelor’s degree with 3 years of experience.

Responsibilities

• Conduct employment readiness assessments and vocational evaluations to identify goals, strengths, and barriers.
• Collaborate with residential and clinical teams on recovery plans.
• Maintain documentation, track outcomes, and ensure regulatory compliance.
• Coordinate referrals with ACCES-VR and other agencies to expand opportunities.
• Develop vocational service plans and provide counseling focused on job readiness and placement.
