Job summary

SpringPath Mental Health Services, a Sevita family member, is hiring a Psychiatric Rehabilitation Practitioner to assist individuals in their journey toward recovery and independence. You will work closely with clients to develop personalized rehabilitation plans, focusing on daily living skills, employment, education, and socialization.

Qualifications

• Bachelor's degree in one of the behavioral sciences from an accredited college or university.
• Minimum of 2000 hours (post Bachelor degree) of verifiable work experience providing services and support to individuals with mental illness.
• Master's degree in one of the behavioral sciences from an accredited college or university preferred.
• Supervisory experience preferred.
• Experience in managing systems, processes, and people.

Responsibilities

• Conduct crisis, medical/behavioral emergency, veteran affairs, and functional assessments.
• Develop crisis/treatment/behavioral plans and goals for clients.
• Write progress notes, summaries, and discharges.
• Lead group and individual counseling using skills teaching, case management, and household management.
• Implement treatment and rehabilitation activities including crisis prevention and medication administration.
• Participate in weekly staff meetings.
• Report significant incidents or accidents.
• Establish trust and provide coaching/learning environment.
• Promote consumer choice and independence.
• Promote community involvement and integration.
