• Meet all agency and contractor background, driving record, and reference checks requirements.

Responsibilities

• Provide 1:1 peer support services to individuals in the community, including home visits and other community locations.
• Link individuals to a range of self-determined supports and services.
• Complete crisis plans and document activities per funding source.
• Maintain accurate documentation and case files as required by policy and regulations.
